RN Case Manager Home Health

Duties and Responsibilities:

  • You will complete an initial assessment of the patient and of the family to determine home health care needs.
  • You will provide a complete physical assessment and history of current and previous illnesses.
  • You will regularly re-evaluate a patient’s nursing needs.
  • You will initiate the plan of care and make necessary revisions as the patient’s status and needs change.
  • You will Initiate the appropriate preventive and rehabilitative nursing procedures.
  • You will counsel the patient and the family in meeting nursing and related needs. You will provide health care instructions to the patient as appropriate per assessment and plan of care.
  • You will identify the process of discharging as a part of the care plan development and implementation prior to discharging the patient.
  • You will act as a case manager and assume responsibility to coordinate patient care.

Requirements:

  1. Graduated from an accredited school of nursing.
  2. Current licensure within the state and an active CPR Card.
  3. Excellent observation, verbal and written communication skills, problem-solving skills, and nursing skills per competency checklist.
  4. Licensed driver with a reliable vehicle. Your vehicle must also be insured in accordance with our organizational requirements.

Job Type: Full-time

Pay: $95,000.00 – $105,000.00 per year

  • Supervisory: 1 year (Preferred)
  • Home Health: 1 year (Required)

License/Certification:

  • RN License (Required)

Ability to Relocate:

  • Philadelphia, PA: Relocate before starting work (Required)
